Spectrum Healthcare Resources has a potential opportunity for a Registered Nurse (RN) - Case Manager at Tyndal Air Force Base near Panama City Florida.

RN Position Details: Ind.Nurse

Responsibilities:

• Coordinate and manage the care of patients through the healthcare continuum.
• Conduct assessments to identify patient needs and develop individualized care plans.
• Collaborate with healthcare providers to ensure timely and effective delivery of services.
• Monitor and evaluate patient progress, adjusting care plans as necessary.
• Provide patient education and support to enhance self-management.
• Advocate for patients to ensure they receive the highest quality of care.

Qualifications:

• Current Registered Nurse (RN) license in the state of South Dakota.
• Bachelor's degree in Nursing (BSN) preferred.
• Certification in Case Management (CCM) is a plus.
• Strong organizational and communication skills.
• Ability to work collaboratively in a team-oriented healthcare setting.

Experience:

• Minimum of 2 years of clinical nursing experience.
• Previous experience in case management or care coordination.
• Familiarity with electronic health records.
