• Coordinates and directs homecare services with physicians, nurses and hospital discharge planning staff to decide patient’s appropriateness for homecare.

Education/Experience

Minimum Required:

• Graduate of an accredited RN program.
• Previous experience in a certified home health agency.

License/Certification

Minimum Required:

• License and current registration to practice as a registered Professional nurse in New York State.

Work Environment

• All work is performed in offices that require working in close quarters with other employees.  The offices are air conditioned/heated.

• Early Adulthood: 18 to 45 Years
• Middle Adult: 46 to 59 Years
• Late Adult: 60 to 79 Years
• Elderly: 80+ Years
