Good Shepherd Hospice is a member of Catholic Health Services of Long Island, an integrated health care delivery system with some of the region's finest health and human services agencies. CHS includes six hospitals, three skilled nursing facilities, a regional home nursing service, hospice and a multiservice, community-based agency for persons with special needs. CHS serves hundreds of thousands of Long Islanders each year, providing care that extends from the beginning of life to helping people live their final years in comfort, grace and dignity.

In this role, you will work under the direction of the Chief Medical Officer for Hospice and Palliative Medicine.  The Nurse Practitioner will provide advanced nursing to patients facing chronic, complex and/or life-threatening conditions in the home setting and/or other community locations.

Responsibilities will include:

  • Performing comprehensive medical and psychosocial assessments
  • Providing skilled and meticulous symptom management, including the prescription of pharmacologic agents and non-pharmacological therapies
  • Formulating, implementing, monitoring, evaluating and modifying palliative care plans with input from interdisciplinary team members and other collaborating care providers
  • Serving as clinical and educational role model and mentor

Qualified candidate must have a Master's Degree in Nursing and be a graduate of an approved Nurse Practitioner Program. NYS RN license, current NYS certification as a Nurse Practitioner with prescriptive privileges, valid DEA registration number and Board Certification as Advanced Practice Hospice and Palliative Care Nurse (APHPCN) within one year of hire required.  Superior critical-thinking, communication and teaching skills will be expected. Ability to perform diagnostic and treatment modalities and interpret scientific studies required of the specialty. Prior experience in home-based services, complex cases and/or palliative care is preferred; some bedside hospital, oncology, ER or ICU experience a plus.
