Please help spread the word about this very important initiative to increase community awareness of Advance Care Planning.  The initiative encourages nurses to lead by example in establishing their own ACP.  The campaign was developed in response to work done at the 2017 Palliative Nursing Summit hosted by the Hospice and Palliative Nurses Association.
