For grant purposes I am only interested in appointments/elections post Leadership Survey from July 2013 - Dec 2014. If a member has already filled out a request for information from the NAC on board service, note with a * by your name to avoid duplication of entries. I am in need of additional information per a RWJF request that is included in the attached document.
