We are very excited to have two very knowledgeable and experienced providers presenting . Kelly Baxter, DNP, FNP-BC, ACHPN has been a nurse for 23 years and a palliative care nurse practitioner for more than a decade. She has been involved in numerous palliative care projects in Rhode Island. She worked for Care New England Health System from 2013-2019 developing an inpatient palliative care service across a 4-hospital health system. During this time, Kelly was chosen as a Practice Change Leader, 2016 Cohort which included being the PI for a John A Hartford Foundation Grant for Improving Transitions of Care for palliative care patients in Rhode Island, $45,000 grant. More recently, she worked for Third Eye Health, a post-acute telehealth provider where she served as the Director of Clinical Compliance and Quality Metrics, as well as the Clinical Coordinator for the Supportive Care Team. In her role, Dr. Baxter developed a palliative care telehealth pilot program which ran in eight Rhode Island nursing facilities with strong patient outcomes. Publication regarding this work is pending. She currently works at the VNA of Care New England providing clinical coverage for hospice patients. Lastly, she is the founder of Baxter Palliative Consulting, a community based palliative care practice. Dr. Baxter is on the Plenary Assembly for National POLST as the Rhode Island representative. She serves as the State Ambassador for the Hospice and Palliative Care Nurses Association in Rhode Island and sits on the End of Life Coalition. Education roles include adjunct faculty for the DNP program, at Salve Regina University, recurring guest speaker at the University of Rhode Island, and preceptor for nurse practitioner students at varied institutions. She is interested in palliative care education, clinical practice, palliative telehealth, as well as research.
Emily Palmer NP is the Clinical Director of Palliative Care at Hebrew SeniorLife, an integrated system of health care, senior living, research, and teaching that serves more than 3,000 Greater Boston seniors each day. She has specific interest in developing innovative models of care to support choice and well-being for community-dwelling older adults. She is a graduate of Wesleyan University and received her Bachelor of Science in Nursing and Master of Science in Nursing from the MGH Institute of Health Professions
