Good morning,
We have partnered with Maricopa County Public Health Department to encourage our community to
complete this impact survey. We need your help. Our commitment is to get 300 surveys completed. Our
grant payment for completing this task is $3000.00
I have attached the flyer which includes the website.
Please encourage your friends, family, anyone who will assist to complete the survey. We are able to
offer a stipend of $5.00 to the person completing the survey.
We also have individual first aid kits, hand sanitizer, and masks to give as added incentives.
We need 185 more individuals to complete survey. This must be accomplished by June 30, 2021.
I don’t need the names of persons completing the survey, but I need statement from anyone submitting
information statement assuring that the survey was completed.
