The NACLI Awards Committee is seeking to honor those nurses who contribute to the Mission and Goals of the Nurses Association of the Counties of Long Island, who carry on the tradition of revered past honorees and who exemplify the criteria of the awards. NACLI Awards will be presented at the Awards Event. The awards and their criteria are listed in the PDF attachment : NACLI AWARDS Nomination Form.

Nominations may be made by NACLI members on the attached official nomination form (Generic PDF for print  or Online fillable PDF) and returned with all supporting documents via email to the NACLI Awards Committee. 

REQUIRED SUPPORTIVE DOCUMENTS INCLUDE:  

1. Statement of reason:  200 to 500 words providing evidence supporting your nomination

2. Two reference letters

3. Nominee's Resume/CV
