The AAHPM & HPNA Measuring What Matters (MWM) Committee is seeking to collect information on quality measurement, quality improvement education, and electronic health record (EHR) needs in the hospice and palliative care field.
We ask that you spare 15 minutes of your time to take a survey and impact your field.

What will we do with the information?
Information collected will inform the next steps for the MWM project and help guide our strategy to empower/improve the measurement environment within our field, especially in light of MACRA and value-based reimbursement. Your individual answers will remain confidential and will be combined with those provided by others to assist AAHPM and HPNA to set priorities and guide decisions.

The survey will remain open for 3 weeks. If you complete the survey, you will be entered to win one of four $25 Starbucks gift cards by simply providing your email address within the survey. All emails addresses will be discarded once the winners have been chosen.
