This session is designed to identify the skill set required for the CNS to influence and provide leadership to meet the needs of patients with chronic disease and identify gaps/disparities in the current care of patients and/or populations with chronic disease. The CNS has unique preparation to care for these complex patients. This session will review how the CNS Core Competencies relate to managing chronic conditions and will identify areas where some CNSs have been able to significantly influence the care of chronic conditions.
