As many of you may be aware, Advance Care Planning (ACP) was a focus area of the HPNA Palliative Nursing Summit.  

As a post-Summit action and in cooperation with the Institute for Healthcare Improvement (IHI), HPNA is pleased to offer a FREE continuing education opportunity to the members of all organizations who participated in the Summit.

This free webinar, “Championing End-of-Life Conversations” on Wednesday, September 27 at 3:30-4:30pm ET. 

Objectives:

1. Describe the origins of The Conversation Project and the need for individuals to have conversations about their end-of-life care wishes.
2. Identify the resources available to help patients and families have the conversation.
3. Recognize some of the frequent issues that arise for individuals when they have these conversations.
