The Palliative Care Nurse functions as a case manager and is a member of an interdisciplinary team that provides holistic and person-centered approach to care. The nurse provides patient and family-centered care throughout the continuum of illness that optimizes quality of life to minimize any symptoms causing the patient physical or emotional distress. The palliative nurse will provide comprehensive care supporting the holistic needs of patients and their caregivers including the assessment and treatment of physical, emotional and spiritual health.

Requirements 1 Holiday shift per calendar year and 1 weekend every 4 to 6 weeks
