A healthcare organization in New York City is seeking a licensed Registered Nurse (RN) to join their growing Community Health team in the Bronx.

About the Opportunity:

• Schedule: Tuesday to Saturday or Sunday to Thursday
• Hours: 8:30am to 5:00pm
• Setting: Field
• Visits: 4-5 per day

Responsibilities

• Conduct in-home and community-based clinical assessments using the nursing process
• Complete and manage UAS-NY assessments and ensure compliance with regulatory standards
• Collaborate with the interdisciplinary team to develop and update personalized care plans
• Administer medications, perform skilled nursing procedures, and monitor patient responses
• Educate members and caregivers on medications, self-care, and health maintenance
• Coordinate care across various settings, including hospitals, SNFs, etc.
• Document all care and assessments accurately and timely

Qualifications:

• 1+ year of Clinical Nursing experience (CHHA, Acute Care, Medical-Surgical, and/or Critical Care)
• Graduate of an NLN-accredited Nursing program
• Current NYS Registered Nurse (RN) license
• Strong communication, clinical judgment, and documentation skills

Desired Skills:

• Bachelor's Degree in Nursing
• Experience with Frail or Elderly populations (training provided if needed)
