Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Patient Programs & Support Services department! The IBCCP Clinical Nurse Navigator provides navigation services for patients requiring breast and cervical cancer screening and treatment services through the Illinois Breast and Cervical Cancer Program (IBCCP). The clinical nurse in this role will support high-risk patients on the cancer continuum of care, facilitating linkage to treatment services at Erie and partner hospitals, and creating patient care plans as needed.

*Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
Main Duties & Responsibilities
- Provides patient navigation for high-risk and newly diagnosed cancer patients.
- Assists patients in understanding diagnosis and treatment options.
- Refers clients to specialists, as necessary
- Participates in yearly IBCCP Women’s Health Conference
- Creates and contributes to monthly and quarterly IBCCP reporting, including running reports in Cornerstone
- Completes other duties as assigned
Qualifications
Education
- Illinois RN licensure required
Skills and Knowledge
Required:
- Bilingual fluency (English/Spanish) is required. As part of the role, the employee may need to speak with and/or assist Spanish-only-speaking patients.
- At least 2 years of experience in community health or with a similar patient population is required
- One year of customer service experience, preferably in a healthcare setting is required.
- Desire to learn how to supervise others is required.
Preferred:
- 2 years of nursing experience
- 2 years of supervision experience
- Illinois RN licensure required.
- Intermediate proficiency in Microsoft Office a plus.
- Experience using Epic EMR preferred
- Familiarity with Cornerstone preferred.
