Pride Health is hiring a Home Health - Case Manager to support our client’s medical facility in West Allis, WI 53227. This is a 13-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization!

Job Summary:

• Home Health - Case Manager. 40 hours per week (Days: 8 am to 4:30 pm, Monday to Friday)

• Wisconsin State RN License or compact state license

• BLS (AHA)

• Active driver's license

• Bachelor's of science in nursing is required

Home Health - Case Manager:

• Perform admission, care planning, case management, and direct patient care in a busy metro home health branch.

• Wound care experience (wound vacs, pleural drains and ostomy care).

• Able to complete peripheral blood draws, central line care, and IV admin through multiple devices (elastomeric, IV pumps).

• Assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans.

• Working with clients, family and friend support networks and health care professionals to put care plans in place.

• Listening to clients’ concerns and providing counseling or intervention as required.

• Recording clients’ progress including charting referrals, home visits and other notable interactions.

• Evaluating clients’ progress periodically and making adjustments as needed to improve outcomes.

Experience required:

• Two years Home Health is required.

• Experience with Oasis required.

• Weekly Travel Pay Range: $2800/week to $3100/week
