Job Description
FUNCTIONAL DUTIES AND RESPONSIBILITIES:
For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Job Description
FUNCTIONAL DUTIES AND RESPONSIBILITIES:
- Demonstrates awareness of individual patient rights
- Provide safe patient care in individual room and completion of nursing assessment:
- Verifies patient identity.
- Obtain and chart vital signs.
- Record and communicates pertinent data relevant to patient care.
- Verifies operative procedure, site, surgeon and consent.
- Note patient’s physical status.
- Verifies allergies, NPO status.
- Checks current history, lab results and results of diagnostic tests.
- Checks for the presence of dentures, contact lens and secure patient’s property.
- Provides direct care to patients to meet their physical, psychological, socio-cultural and spiritual needs:
- Identifies religious preferences.
- Emotional support and comfort measures.
- Gives information to patient and family as needed.
- Ensure and respect patient’s rights, privacy and dignity.
- Identify the patient’s ability to understand and preferred language.
- Receives reports from recovery room, radiology, mammography and GI using SBAR communication sheet and give reports to relief staff member during break and shift change.
- Assures responsibility and accountability of assigned unit:
- Acts as a Charge Nurse.
- Exercise sound judgment in decision making
- Help with maintaining records and statistics
- Assess patients post operatively and ensure patient safety upon discharge.
- Check emergency cart/RRT box daily.
- Evaluate patient response to care via follow –up eg. Telephone calls.
- Participates in Performance Improvement activities.
- Demonstrates ability to function as Pre-Admission Screening Nurse:
- Ensure that all PAT ordered by physician have been completed and results have been received.
- Complete vital signs /height and weight of patient.
- Complete Medication Reconciliation.
- Review all test results for completion and abnormalities.
- Contact physician to discuss potential delays if abnormalities are discovered.
- Review patient’s medical record to identify any pre-existing conditions that may require stabilization.
- Determine that all necessary consults have been obtained or scheduled.
- Perform or delegate collection of blood samples for lab test ordered.
- Perform or delegate EKG tracings according to anesthesia requirement.
- Maintain log of daily activities of patients and completion of billing sheet.
- Complete and document electronic pre-admission visit note.
- Assist in the development of staff unit-based orientation and education:
- Orientation of new personnel.
- Delegates assignments and task to support staff including Clerks Associates,
- Formulation and interpretation of procedures, standards and techniques.
- Assumes accountability and responsibility for his/her own work performances:
- Observe assigned work hours and schedules.
- Provide availability for emergencies.
- Assist with inventory control and check expiration dates.
- Maintains open communication with staff and surgeons.
- Recognizes symptoms of cardiac arrest, respiratory distress and/or other impending emergencies and initiates the necessary action.
- Applies principles of proper body alignment during transfer and positioning of patients.
- Actively participates in unit specific performance improvement projects
- Delegates and ensure completion of assignments/task by support staff.
- A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
