Position Description
Compass Healthcare Consulting & Placement is conducting a search for a Senior RN Manager of Intake, Registered Nurse. Position is with a Certified Home Health Agency (CHHA) located in Queens, NY. Qualified candidates will have Prior Hospital, Discharge Planning, RN Liaison, Intake RN, Field RN Supervisor, RN Case Management or Coordinator of Care experience within a Certified Home Health Agency (CHHA) or Hospital, will understand the Intake process and will want to join a great team! Position is located in office, in Queens, NY. Will train RN with outgoing personality who understands Home Care Liaison and Discharge Planning role.
Senior RN Manager of Intake - Registered Nurse
The Senior RN Manager of Intake is an essential component of the certified agency team. The Senior Manager is responsible to supervise, instruct and guide intake clinicians and auxiliary personnel in referral intake and follow-through. Designs and implements efficient work processes to ensure sound communication between departments, referral sources and physicians.
Qualifications:
- NYS RN license
- Certified Home Health RN experience a plus!
- Good verbal and written communication skill
- Understanding of Home Health Intake and Admissions Process including Discharge Planning
- Excellent clinical skills
- Computer literacy
Responsibilities:
- Receives information from referral sources regarding patients that may require home health care services.
- Interviews the patient and family/caregiver in planning for discharge, and family meetings are arranged as necessary. Assesses for environmental barriers and/or needs to the patient’s release from the hospital. The findings are documented in the referral form and communicated to the appropriate Home Care Coordinator.
- Obtains an authorization from MD for home care services.
- Accepts patients for home health services in accordance with the CHHA policies and procedures.
- Registers all referrals in the referral log registry.
- Participates in the development of the plan of treatment for each patient admitted for home health care services and identifies realistic goals based on the patient’s needs prior to the patient’s admission for home health care.
- Involves the physician, patient, and family/caregiver in formulating the plan of treatment. The plan of treatment reflects input from the physician and the patient.
- Informs the appropriate Home Health Care Coordinator of the patient’s home care needs on or before the day of discharge.
- Ensures that patient charting is comprehensive and reflects a trail of coordination between the physician and/or referring entities and other disciplines involved in the care of the patient.
- Serves as a resource person to hospital personnel, patients, and family/caregivers.
- Liaises with Business Development staff to ensure referrals are screened and referred timely.
- Liaises with Clinical Staff and Schedulers to ensure referrals can be processed and seen timely.
- Contacts referral sources on a regular basis to maintain a positive customer service relationship.
- Communicates with referral sources to review eligibility for agency services and assists referring agents with alternate plans for ineligible patients.
- Works in collaboration with Intake Director to identify deficiencies in the workflow.
- Demonstrates sound judgment by taking appropriate actions regarding suspected violation of corporate compliance regulations.
