Job Description

One of the largest public health nonprofit in New York City, is seeking a Nurse Home Visitor (RN) for its Nurse-Family Partnership (NFP) program in Northern Queens. This nationally recognized, evidence-based program provides critical home visiting services to high-risk, low-income, first-time mothers from pregnancy through their child’s second birthday. The Nurse Home Visitor delivers compassionate, patient-centered care in a community-based model, helping to reduce health disparities and improve maternal and child outcomes.

Responsibilities/Job Duties:

• Conduct home visits for eligible pregnant and parenting women and their families
• Provide case management and education from pregnancy through age two of the child
• Carry a caseload of at least 25 clients
• Apply motivational interviewing, reflection, and nursing process in care delivery
• Assess physical, emotional, social, and environmental needs
• Assist families with goal-setting and progress tracking
• Provide referrals and coordinate services with other community providers
• Complete Medicaid billing documentation
• Participate in staff meetings, training, and clinical supervision

Minimum Education and Experience Qualification Requirements:

• BSN required (or Associate's Degree with current BSN enrollment)
• Current NYS Registered Nurse license in good standing
• CPR certification required
• Minimum 2 years’ experience in maternal/child health, public health, home visiting, or behavioral health preferred
• Bilingual English/Spanish preferred
• Valid driver’s license and insured vehicle required
• Flexibility to work evenings/weekends as needed
