Position Details:

• Location: Queens, NY
• Employment Type: Full-Time
• Salary: $100,000 annually
• Schedule: Monday – Friday, 8:30 AM – 5:00 PM

Qualifications:

• Associate or Bachelor’s Degree in Nursing
• Active NY State RN license
• Minimum 6 months of clinical nursing experience
• Skilled in care planning and clinical documentation
• Strong communication, organization, and teamwork skills

Key Responsibilities:

• Conduct home care assessments and reassessments using tools such as OASIS
• Develop and implement individualized Plans of Care in collaboration with the physician and supervisor
• Deliver skilled nursing services including specialized treatments and preventive procedures
• Monitor patient conditions and update treatment plans as needed
• Educate patients and caregivers on health needs and care procedures
• Supervise and document performance of home health aides
• Ensure compliance with infection control, confidentiality, and facility protocols
• Maintain accurate and timely documentation according to facility policy
• Participate in performance improvement initiatives and support client rights
