Job Title: Registered Nurse – Adult Care

Location: New York, NY 10004

Schedule: Monday–Friday, 9:00 AM–5:00 PM (40 hours/week)

Type: Full-Time

Overview:

We are seeking a compassionate and skilled Registered Nurse (RN) to provide community-based care to adults across New York City, including individuals in group/private homes and those experiencing homelessness. Under the supervision of the Assistant Director of Nursing, the RN will deliver direct nursing services, develop care plans, and coordinate with interdisciplinary teams to support patient health and well-being.

Qualifications:

• Valid NYS RN license and BLS certification (AHA)
• Bachelor’s degree in Nursing (if licensed after July 1, 2020)
• Minimum 1 year RN experience
• Strong clinical, communication, and critical thinking skills
• Proficient in Epic, Microsoft Office, and electronic documentation

Key Responsibilities:

Conduct comprehensive assessments and develop individualized care plans

Provide skilled nursing care (e.g., wound care, medication education, diabetic care)

Maintain accurate documentation (including OASIS) and care notes

Collaborate with physicians and health team members

Travel to patient locations citywide
