Providence at Home with Compassus

Take your nursing career to the next level with Providence at Home with Compassus—a collaboration between two leading providers in home-based care. We are currently seeking an experienced Home Health RN to join our team in delivering compassionate, outcome-driven care directly in patients’ homes.

Position Overview

As a Home Health RN, you will:

• Work independently and within an interdisciplinary team.
• Conduct comprehensive patient assessments.
• Develop and implement individualized care plans.
• Provide evidence-based interventions under physician orders.
• Deliver care with a focus on safety, professionalism, and empathy.

This role is rooted in community health nursing and emphasizes the importance of patient-centered, in-home care.

Key Responsibilities

• Assess and respond to the developmental and clinical needs of patients.
• Interpret patient behavior and self-reports to guide care.
• Collaborate with families and healthcare teams to build care plans.
• Document care activities accurately and promptly.
• Support orientation of new staff and contribute to department activities.
• Stay current through continuing education and training.
• Foster open communication and healthy team relationships.

Qualifications

• Required: RN license in state of employment; BLS certification (AHA).
• Preferred (WA/AK): 1 year of recent experience in home health, acute, or skilled nursing care.
• Education: Associate’s or Bachelor’s degree in Nursing from an accredited school.

Physical & Work Environment

The role requires regular mobility, lifting (up to 50 lbs), and use of clinical tools. Exposure to typical healthcare environments, including infectious diseases, is expected.
