Position Description

Job Summary

We are seeking a compassionate and self-driven Registered Nurse (RN) Case Manager to support clients in the Maplewood, MN area. This home-based role centers on case management and caregiver support, rather than heavy bedside care, making it an ideal opportunity for nurses seeking improved work-life balance.

In this position, you will visit clients in their homes—many of whom need assistance with daily living activities, medication administration, and catheter care. You will oversee caregivers, perform client assessments, offer training and supervision, and respond to any changes in condition. With a flexible schedule and lighter physical demands, this role is well-suited for nurses transitioning away from acute care environments.

Key Responsibilities

  • Conduct routine assessments for comprehensive clients in line with state guidelines
  • Oversee caregivers performing tasks such as medication administration and catheter care
  • Provide training and ongoing supervision to caregiver staff
  • Visit clients primarily during assessments or in response to condition changes (e.g., falls)
  • Complete documentation through an electronic system (iPad provided)
  • Work collaboratively with another RN to manage a shared caseload of approximately 45 clients (20–25 each)
  • Must-Have Qualifications

    • Active RN license in the state of Minnesota
    • Strong assessment and clinical decision-making skills
    • Reliable transportation and a valid driver’s license (mileage reimbursed from office)
    • Comfortable traveling to client homes and managing a flexible schedule
    • Familiarity with basic home care procedures, including medications and catheters

    Nice-to-Have Qualifications

    • Experience in home health, hospice, or community care
    • Background in caregiver training, mentorship, or staff development
    • Previous experience in case management or care coordination
