Job Title: Licensed Practical Nurse (LPN)

What You’ll Do

• Deliver skilled nursing care based on physician orders and supervised RN direction
• Administer medications, treatments, and procedures with precision and empathy
• Monitor client health and report changes to ensure timely, appropriate interventions
• Educate clients and families to empower them in their care
• Collaborate with an interdisciplinary care team to create and adapt care plans
• Maintain accurate, timely documentation that reflects your expert care
• Stay up to date on best practices, standards, and regulations—because excellence matters

What You’ll Need

• Graduate of an accredited LPN program
• Current, active state licensure
• 1+ year of experience in an acute care setting (home care preferred)
• Certification to administer medications (NAPNES or equivalent)
• Reliable transportation and a valid driver’s license
• A heart for service and strong communication skills
  
  

Physical Requirements

• Frequent walking, lifting (up to 35 lbs), and assisting with patient mobility
• Ability to manage shifting priorities and a fast-paced care environment
• Flexibility to travel between client homes and work sites
