As a Home Health Registered Nurse (RN), you’ll deliver skilled nursing care directly in participants' homes, enhancing quality of life through personalized care and strong community-based support.

About the Role - Conduct in-home nursing assessments to determine care needs, goals, and preferences. Collaborate with an interdisciplinary care team to develop and implement participant care plans. Provide skilled nursing interventions including medication administration, wound care, and other physician-ordered treatments. Educate participants and caregivers on disease management, prevention, and self-care. Monitor and track home health visits in coordination with the Home Health Manager. Facilitate care transitions between hospitals, nursing facilities, and home settings.

Responsibilities

Qualifications - Graduate of an accredited nursing program. Active, unencumbered RN license in the state of California. At least 1 year of clinical nursing experience (experience with elderly or home health preferred). Reliable means of transportation to conduct home visits.
