Qualifications

• Education: Associate or Bachelor’s Degree in Nursing
• Licensure: Active RN license in New York State
• Experience: At least 6 months of clinical nursing experience
• Skills:
• Care planning and nursing documentation
• Strong communication, organization, and critical thinking
• Ability to work independently and within a team

Key Responsibilities

• Perform comprehensive home care assessments (including Oasis if required)
• Collaborate with physicians, supervisors, and families to develop individualized care plans
• Deliver skilled nursing care per treatment plans, including treatments requiring advanced nursing skills
• Monitor and report changes in patients’ condition; update care plans accordingly
• Educate patients and caregivers on care procedures and wellness strategies
• Maintain accurate and timely documentation in accordance with facility policies
• Supervise home health aide staff and ensure compliance with care standards
• Adhere to infection control, confidentiality, and patient rights protocols
