Job Title: Case Manager RN

Job Type: Contract (7 months)

Location: Gastonia, North Carolina 28052

Hours: 8:00 AM - 5:00 PM (Monday - Friday)

Rate: Negotiable

Primary Responsibilities

• Ensure accurate and complete documentation.
• Communicate and collaborate effectively with the interdisciplinary care team.
• Support providers and care teams in various care coordination activities.
• Assist in benefit determination related to Medicare Part A, Part B, and Part D.
• Collaborate with the primary advanced practice clinician (APC) and facility to address patient condition changes.
• Facilitate therapy coordination and ensure seamless integration into patient care plans.
• Participate in facility-based meetings to align care strategies.
• Schedule and participate in family conferences, team meetings, and case presentations.
• Assist with training coordination, quality measure tracking, and compliance.
• Develop and lead in-service training sessions for selected audiences.
• Support providers in maintaining accurate documentation, coding, medication reconciliation, nursing rounds, assessment, and patient records.
• Comply with state RN practice laws and facility protocols when writing verbal orders.
• Aid in managing acute medical situations and chronic condition exacerbations, including diagnostics, testing, and interventions.
• Assist members throughout short-term sub-acute rehabilitation episodes.
  
  

Required Qualifications

• Active and unrestricted RN license in the state of residence (or ability to obtain by start date).
• Associate degree or higher in Nursing (RN).
• Basic Life Support (BLS) certification.
• Ability to perform physical tasks, including lifting a 30-pound bag and conducting physical assessments.
• Reliable access to transportation.
• Full COVID-19 vaccination, in accordance with all federal, state, local, and client regulations (proof required before employment).
• Ability to pass a pre-employment drug screening.
  
  

Preferred Qualifications

• 1+ year of clinical experience (long-term care experience preferred).
• Experience caring for patients with complex behavioral, social, or functional needs.
• Strong understanding of geriatrics and chronic illness management.
• Proficiency in medical documentation, both written and electronic.
• Excellent communication, time management, and teamwork skills.
• Ability to work collaboratively across diverse care settings.
• Reliable transportation and ability to travel between facilities.
