This position provides direct nursing care for members of Yale Health Acute Care/OB/GYN. The primary role is to provide acute and on-going care for the Yale Health OB/GYN member population. Role includes significant phone triage as well as assessment of walk in patients, care coordination and direct nursing visits.
The position will operate as a Team Lead within OB/GYN and provide daily leadership in collaboration with the Nurse Manager which will include: oversight of medical assistants on the team including performance monitoring, coaching, evaluation and training; delegation of daily medical assistant assignments; approves PTO for team clinicians and medical assistant staff in accordance with departmental time off policies; ensuring minimum staffing requirements for team; works with SAA to develop and provide weekly coverage assignments for In Basket and patient messages for clinician who are out of the office, including PTO, Comp Time, CME, Sick Time and LOA’s. Ensures a safe environment in the department, care of equipment; and collaboration with Department Leadership for initiatives designed to improve delivery of care.
Reporting to the Manager of Ob/GYN, assumes responsibility for all nursing duties that support the OB/GYN Clinical Care team. This includes direct nursing care for OB/GYN patients, and patient centered direct care for routine care and acute/episodic needs of the OB/GYN member population. Clinical judgment and decision making in accordance with clinical practice guidelines and protocols and clinician orders, including follow up, preventative health, maintenance, teaching and education. Responsible for evaluation and disposition of scheduled and same day- walk in patients. Significant emphasis on telephone nursing/ triage, as well as case management and care coordination. Will function as a liaison to clinicians regarding triage/clinical care decisions and facilitate patient flow. Anticipate patient care needs based on patient assessment and prioritize care. Foster ongoing improvement of access to services. Nursing responsibilities includes the Visit RN, Team/Care Coordination RN, Walk-in Triage, and Telephone Triage RN roles, as well as handling of department My Chart Messages. Assure appropriate documentation of patient encounters, including patient teaching, self-care advice, and coordination of health services as indicated. Oversight and supervision of support staff on a care team and the ability to delegate to clinical and support personnel, within their scope of practice/responsibility. Identify education needed to assure competent delivery of patient care for oneself and others in the department. Responsible for performance evaluations, training, coaching and orientation of members on a care team. Participate in departmental quality improvement activities. May assume charge nurse responsibilities as indicated. Communicate with all health care clinicians to ensure appropriate on-going care. Responsible for the maintenance of a comfortable, orderly, safe and clean environment. Additional projects and responsibilities as indicated, including outreach and population health initiatives. Performs and/or supervises all CLIA waived testing performed in department(s).

Essential Duties
Bachelor’s Degree in Nursing. Two years experience or an equivalent combination of education and experience.
Required Skill/Ability 1
Demonstrated clinical ability in Maternal/Child Health and or labor and delivery. Proficiency in fetal monitoring and assessment of the OB patient. Current CPR.
Required Skill/Ability 2
Triage skills with ability to support a high-volume practice. Strong assessment skills.

Ability to function in a diversified fast paced environment. Ability to multi-task with thorough and complete follow through including written documentation.

Essential Duties
- Performs physical assessments of patients, including triage duties and advises on treatment or course of action by having a thorough knowledge of nursing care standards and principles. 2. Identifies and evaluates nursing services problems and develops and implements appropriate programs and services, including assessing staffing needs and arranging coverage as needed. 3. Develops, implements, and manages nursing care plans in collaboration with physicians. 4. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 5. Provides leadership, guidance, direction to clinical staff, and collaborates with nursing at all levels. 6. Determines standards for quality assurance and infection control procedures; trains staff in universal and isolation precautions in the area of infection control. 7. Performs diagnostic and therapeutic measures in collaboration with physicians. 8. Evaluates the quality and effectiveness of nursing practice and nursing services. 9. Records patients’ medical histories and symptoms, performs diagnostic tests and analyzes results, operates medical machinery, administers treatment and medications, and coordinates with patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines.
