RN Case Manager

StartDate: 8/1/2025 Pay Rate: $2000.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an Acute Rehab setting

MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 1 year acute case manager experience

PREFERRED QUALIFICATIONS – 2 Years Acute Case Manager Experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Normal business hours & Every other weekend

SYSTEMS – Rehab Connect/ProDoc

START DATE – 8/1/2025
