Registered Nurse (RN) Case Manager

Location: Bellevue, WA

Schedule: Monday – Friday, 8:30 AM – 5:00 PM

Overview:

Medix is partnering with a nonprofit community health organization to hire two dedicated and experienced RN Case Managers. This role is responsible for planning, organizing, and providing nursing services to clinic patients in collaboration with healthcare providers and the broader care team. The RN Case Manager will work closely with patients who have chronic disease conditions and/or complex health needs, providing direct care, education, and support to help patients achieve optimal health outcomes

Training Period:

• 90-day training period, with close support from the Nurse Manager (may be shortened for highly experienced candidates).

Preferred Qualifications:

• Experience with EPIC or other Electronic Medical Records (EMR)
• Experience with Geriatric populations

Soft Skills & Attributes:

• Excellent time management and organizational skills
• Collaborative and team-oriented mindset
• Ability to work independently
• Proficiency in computer use and documentation

Additional Information:

• Patient Population: Primarily older adults (65+), with occasional younger patients
• Patient Volume: See 15–30 patients weekly; frequency depends on condition severity
