Position Description
Pride Health is hiring a compassionate and highly organized Case Management RN to join our interdisciplinary care team in Jacksonville, FL! This facility-based position plays a critical role in care coordination, benefit navigation, clinical documentation support, and patient advocacy across multiple skilled nursing facilities.
Shift: 8a-5p Monday-Friday
Pay: $40-$45 hourly
Contract Length: 6+ months
*Travel is required between 3 assisted living facilities in Jacksonville, FL*
Primary Responsibilities:
- Ensure accurate, complete, and timely documentation of patient care.
- Collaborate closely with interdisciplinary team members, including providers, facility staff, and advanced practice clinicians (APCs).
- Support providers with care coordination, benefit determination (Medicare Part A, B, and D), and therapy planning.
- Identify and respond to patient changes in condition in coordination with providers and facility teams.
- Participate in facility-based meetings, family conferences, and team case reviews.
- Support training, quality measure compliance, and internal in-services.
- Facilitate documentation and coding accuracy, nursing rounds, medication reconciliation, and assessments.
- May transcribe and document provider verbal orders per state RN practice laws and facility guidelines.
- Assist with acute situations, chronic condition exacerbations, diagnostics, and rehabilitation episodes.
Required Qualifications:
- Active and unrestricted RN license in Florida (or compact)
- Associate degree or higher in Nursing
- Certified in Basic Life Support (BLS)
- Access to reliable transportation
- Full COVID-19 vaccination in compliance with federal and client guidelines
- Ability to pass a pre-employment drug screen
Preferred Qualifications:
- 1+ year of clinical experience (long-term care setting preferred)
- Experience working with patients with complex medical, behavioral, or social needs
- Familiarity with geriatrics and chronic illness care
- Proficiency in electronic medical records (EMR) and medical documentation
- Strong communication and time management skills
- Comfort working in dynamic, multidisciplinary environments
