JOB SUMMARY

Family Services of Westchester is currently seeking a F-ACT Registered Nurse for our Restorative Justice Program in Yonkers, NY. FACT (Forensic Assertive Community Treatment) supports adults with severe mental illness who are involved with the criminal justice system.

JOB TYPE: Full-Time

PAY: $110,000/Year

SCHEDULE: 37.5 hours per week

RESPONSIBILITIES

• Medical monitoring & Support: provide medical oversight , including physical health assessments and medication management for participants.
• Collaboration: work with the clinical coordinator an PNP to ensure physical conditions are appropriately managed and within the treatment plan.
• Work collaboratively with the team to meet participants in their homes/community to assess physical health as appropriate. Complete timely case notes in designated EHR.
• Present with at least 3 years of experience with individuals with mental health and substance use disorders in a forensic setting.
• Strong clinical assessment and health management skills, particularly in managing chronic illness and medication adherence.

QUALIFICATIONS

• Bachelor's Degree of Science in Nursing (BSN)
• 3 years of experience with individuals with mental health and substance use
