A rehabilitation services organization on Long Island is currently seeking Licensed Practical Nurses (LPN) to join one of their skilled nursing facilities in Suffolk County.

About the Opportunity:

• Setting: Skilled Nursing Facility
• Shifts: Days, Evenings, Nights
• Schedule: 4-5 shifts per week
• Hours: 7am to 3pm or 3pm or 11pm or 11p-7a (Open to double 16 hour shifts)
• Location: Yaphank, New York

Responsibilities:

• Assist residents with activities of daily living (ADLs): including bathing, dressing, grooming, toileting, and feeding.
• Monitor and report changes in residents' conditions: such as vital signs, behavior, or mobility changes to nurses or supervisors.
• Provide mobility support
• Maintain a clean and safe environment: including changing bed linens, cleaning resident areas, and ensuring equipment is sanitized.
• Document care provided: accurately record daily tasks and any observed changes in the resident's physical or mental condition.
  

Qualifications:

• Graduate of an accredited School of Nursing
• NYS Licensed Practical Nurse (LPN)
• Solid assessment and documentation skills
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
