Position Description

Pride Health is hiring a Case Manager RN to support our client’s medical facility in Albuquerque, NM. This is a 13-week assignment (with the Possibility of extension or conversion) and a great opportunity to start working with a top-tier healthcare organization!

Job Title: Case Manager RN

Location: 1100 Central Ave. SE, Albuquerque, New Mexico, 87106

Shift: 8-Hour Days (5x8 hrs per week)

Contract Duration: 13 weeks with possible extension

Guaranteed Hours: 40 hours per week.

Job Summary:

The Case Manager/Social Worker is responsible for interviewing and assessing patients, families, and designated individuals within 48 hours of admission. This role focuses on understanding financial, emotional, physical, social, functional, and healthcare needs to recommend discharge plans, manage expectations, reduce readmission risks, and resolve discharge barriers.

Key Responsibilities:

  • Communicate with physicians to ensure medical necessity documentation is complete.
  • Elevate cases to the Supervisor/Medical Director for patients not meeting criteria.
  • Educate patients, families, and the interdisciplinary team about post-acute care options and care coordination.
  • Develop, implement, and monitor discharge plans in collaboration with the team.
  • Facilitate timely intervention to address discharge needs and prevent delays.
  • Participate in multidisciplinary rounds and address target length of stay with healthcare providers.
  • Monitor patient length of stay and escalate issues to the medical director as needed.
  • Document assessments, status, and interventions in the medical record.
  • Provide comprehensive clinical social work and case management across both inpatient and outpatient settings.
  • May require floating between departments and alternate locations within a 20-mile radius.

Requirements:

  • Minimum of two (2) years of current experience in the specialty.
  • Must have completed at least one 13-week travel assignment.
  • Associate’s degree in Nursing required.
  • State of New Mexico or Compact State Nursing License.
  • BLS certification through the American Heart Association, American Health and Safety, or Red Cross.

Preferred Qualifications:

Previous experience in a fast-paced, team-oriented environment.

Other:

On-call, weekend, and holiday shifts may be required.

Must be flexible and willing to float between departments as needed.
