Position Overview:

Greenlife Healthcare Staffing is seeking an experienced Registered Nurse MDS Assessor to join a Skilled Nursing Facility in New York, NY. This role involves conducting comprehensive patient assessments, ensuring regulatory compliance, and coordinating interdisciplinary care plans to optimize resident outcomes in a long-term care setting.

Qualifications:

  • Education: Associate’s or Bachelor’s degree in Nursing (ADN/BSN).
  • Licensure/Certifications:
  • Active New York RN license.
  • MDS Assessor Certification (required).
  • Experience: 1–2 years in long-term care with MDS coordination experience.
  • Soft Skills: Attention to detail, strong communication, and ability to lead interdisciplinary teams.

Key Responsibilities:

  • Intake and evaluation process of patients in long-term care facilities.
  • Up-to-date assessments of the patient and oversee the interdisciplinary assessment for all nursing home patients.
  • MDS coordinator monitors patient care by assessing procedures.
  • Speaking with patients and recording medical codes.
  • MDS coordinators also enter medical codes and are involved in medical billing processes, ensuring they maintain accurate medical records.
