Position Description
The Strong Children Wellness (SCW) Community Health Equity Fellowship is a one-year training program for Family Nurse Practitioners (FNPs) who are committed to health justice and serving under-resourced communities. Fellows work in our integrated, team-based primary care practice and receive mentorship from SCW’s leadership team who are experts in health equity, systems transformation, and community-based care.
Fellows care for medically and psychosocially complex children and adults, build trust and continuity with their patients, and learn to lead community-engaged health equity projects in partnership with trusted local organizations. Designed to support your growth as both a clinician and systems change leader, this fellowship equips you to transform care delivery for underserved populations.
- We welcome applications from licensed Family Nurse Practitioners who have:
- A strong commitment to advancing health equity
- Experience working with psychosocially complex and/or
low-income populations - Interest in community partnership and health systems change
- Passion for working in under-resourced communities
- Interest in working within multidisciplinary settings that address physical health, mental health and social needs
- *All fellows must commit to a minimum of one additional year of employment at SCW after completion of the fellowship

The fellowship is designed to be immersive, balancing structured learning with hands-on, community-based practice.
