Lead RN Case Manager – Discharge Planning

Location: Holy Cross Health | Silver Spring, MD

Employment Type: Full - Time

Position Purpose

Holy Cross Health is seeking a Lead RN Case Manager to oversee discharge planning efforts and serve as the senior clinical resource within our Case Management department. This role partners with leadership to develop and implement best practices that enhance patient care, ensure compliance, and support successful transitions from hospital to home or other care settings.

What You Will Do

  • Lead and mentor case managers with a focus on effective, patient-centered discharge planning
  • Oversee orientation and ongoing competency development for case management staff
  • Support quality improvement initiatives and clinical documentation integrity
  • Collaborate with interdisciplinary teams to remove discharge barriers and optimize care transitions
  • Act as a resource for complex case reviews and regulatory compliance
  • Promote consistent communication with medical staff, nursing, and ancillary departments

Minimum Qualifications

  • RN licensed in Maryland
  • BSN required; Master’s degree or Case Management certification preferred
  • Minimum two years of hospital-based case management experience, including exposure to quality improvement and documentation review
  • Strong communication, leadership, and interpersonal skills
  • Knowledge of hospital reimbursement and regulatory guidelines
  • Proficiency in clinical systems and data analysis

Pay rate: $43.04 - $64.56

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
