Position Description
Pride Health is hiring a Case Manager RN to support our client’s medical facility based in Wake, Durham Counties in NC. This is a Contract to Hire opportunity and a great way to start working with a top-tier healthcare organization!
Job Details:
- Ensure accurate and complete documentation.
- Communicate and collaborate with the interdisciplinary care team
- Assist the provider/team with various care coordination activities
- Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
- Responsible for collborating with the primary advanced practice clinician (APC) and the facility to identify and respond to patient changes in patient condition
- Assist the provider/team with therapy coordination
- Participate in facility-based meetings
- Schedule and participate in family conferences, team meetings, and team case presentations
- Coordination of training, tracking, and compliance with quality measures
- Assist in creating and conducting in-services for selected audiences
- Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
- May write verbal orders from the Provider in the chart in compliance with the state RN practice laws, and in compliance with individual nursing facility practices
- Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions
- Assist in managing members during a short-term sub-acute rehabilitation episode
Qualifications:
- Active and unrestricted license in the state which you reside, or ability to obtain by start date
- Associate degree or higher in Nursing (RN)
- Certified in Basic Life Support
- Ability to complete physical requirements, including lifting a 30-pound bag and the ability to perform physical assessment
- Access to reliable transportation

Certification required:

Additional Information:
Start & end times of shift: 8-5pm
Days of the week they will work: Mon-Fri
Where they will travel: Wake, Durham Counties in NC
*Offered pay rate will be based on education, experience, and healthcare credentials.
