Position Description

Duties Description

  • The RN 1 will ensure appropriate information is provided for all deaths. This includes ensuring incidents are classified correctly, and a thorough description of each incident has been received, and that required notifications to entities such as law enforcement are completed as appropriate. The RN 1 alerts authorized personnel to any medical or health concerns that may be revealed while the death investigation is being reviewed; this includes notifying the New York State Justice Center (NYJC) if Abuse/Neglect is suspected.
  • The RN 1 will record and track the deaths in the Mortality database after determining the referral status.
  • The RN 1 will: review investigations and investigative records; process investigative and related incident records as appropriate and required; interact with oversight agencies and provider agencies - relative to investigative records, procedures, and processes, and follow up as necessary.
  • The RN 1 will conduct initial review of individual incidents of mortality, that are reported in the Incident Report and Management Application (IRMA), and additional incidents as assigned. The RN 1 will make appropriate recommendations for each incident using clinical judgment and criteria developed by the Central Mortality Review Committee.
  • The RN 1 will read, review, and interpret completed investigations submitted, medical/hospital records, laboratory values, nursing notes, health charts, medication regimen and all other health related data so they may submit monthly summaries to the Mortality Review Committee based on their findings. The RN 1 will identify deaths that require further review by OPWDD's Mortality Committee. For mortality incidents recommended for review and closure, the RN 1 will submit to the Local and Central Mortality Review Committees, a summary of each case and aggregated data regarding all cases recommended for closure.
  • The RN 1 acts as a health liaison with members of the Incident Management Unit (IMU) and the Agencies served, by researching and answering complex medically related questions as they arise. This includes but is not limited requesting missing or incomplete records such as Autopsy Reports, Death Certificates, Ambulance Reports, Hospital Records, etc.
  • The RN 1 will work in conjunction with the IMU Investigations Team, as well as the Statewide Committee on Incident Review (SCIR) and Local and Central Mortality Committees to ensure a strong, coordinated approach is taken in response to any abuse allegations or other serious incidents that may occur within state-operated services.
  • The RN 1 will provide ongoing technical assistance and direction to OPWDD; NYJC staff; and state operated and voluntary provider agencies in the area related to deaths. The RN 1 will provide medical and health related answers to any inquiries made, as appropriate.
  • The RN 1 will develop an understanding of the Part 624 and 625 (14 NYCRR) handbook, commentary and FAQs and provide advice and direction as appropriate.
  • The RN 1 will provide DQI staff (Bureau of Program Certification and Continuous Quality Improvement) with pertinent information for monitoring and evaluation and alert Bureau of Program Certification staff to significant untoward events for immediate follow up, as needed.
  • The RN 1 will assist in the development of appropriate and standardized trend analysis reports for the Commissioner's and Leadership Team's review - related to deaths.
  • The RN 1 will maintain a proficiency for the use of all MICROSOFT Office Suite applications.
  • The RN 1 will learn and maintain a proficiency for the use of all OPWDD and NYJC databases and all systems used.
  • The RN 1 will learn and maintain a proficiency for the use of all processes, including administrative processes, needed to fulfill their tasks.
  • In addition, the RN 1 will perform other duties as determined to be necessary for effective mortality or morbidity review.

Minimum Qualifications

NON-COMPETITIVE MINIMUM QUALIFICATIONS:

Possession of a license and current registration as a registered professional nurse in New York State.

COMPETITIVE MINIMUM QUALIFICATIONS:

There is currently no Civil Service eligible list for this title.

REASSIGNMENT QUALIFICATIONS:

Current Office for People with Developmental Disabilities employees with one year of permanent, contingent permanent, or 55 B/C service as a Registered Nurse 1. If you have less than one year, you must be currently reachable on the eligible list.

TRANSFER QUALIFICATIONS:

Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1 or 70.4 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

55 B/C QUALIFICATIONS:

  • Possess the minimum qualifications for this title as established by the Department of Civil Service and listed on the most recent exam announcement.

  • Have a current Eligibility Letter from the NYS Department of Civil Service, www.cs.ny.gov/rp55/, and be able to provide it to the Human Resources Office upon request.
  • Please note, the posted Hiring Salary includes the sum of the statutory hiring rate for Grade 18 ($66,951), along with any special salary treatments in effect for this position in this location, such as a Geographic Differential ($20,000) and Increased Hiring Salary ($9,196).

  • NYS HELPS

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non- competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

  • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee
  • Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume/cover letter.
  • All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
