Pride Health is currently hiring a contract-to-hire Case Manager RN for our client in Adams county, PA! Join a dynamic care coordination team and help improve outcomes for patients in post-acute and long-term care settings. This is an excellent opportunity for Registered Nurses to transition from bedside care to a Monday–Friday schedule with nights and weekends off.

Shift: M-F 8a-5p

Pay: $40-$45 hourly

Contract Length: 6+months, contract to hire

*Required to travel to multiple assisted living facilities within Adams and Cumberland Counties*

Duties:

• Ensure accurate, timely, and complete clinical documentation
• Collaborate with interdisciplinary teams to support patient care plans
• Assist providers with benefit determination (Medicare Part A, B, and D)
• Monitor and respond to changes in patient condition in partnership with APCs and facility staff
• Coordinate therapy services, nursing rounds, and family/team conferences
• Participate in facility-based meetings and case presentations
• Support training and quality compliance initiatives, including in-service education
• Promote accurate provider documentation, coding, and medication reconciliation
• Facilitate short-term sub-acute rehab and chronic care management
• May transcribe verbal orders in compliance with RN practice laws and facility protocols

Requirements:

• Active and unrestricted PA or compact RN license
• Associate's degree in Nursing or higher
• BLS Certification
• Access to reliable transportation
• Full COVID vaccination is required
• 1 year of LTC experience preferred
• Understanding of Geriatrics and Chronic Illness
