Job Description

The nurse navigator functions on a multidisciplinary team as an advocate, educator, and communicator for the oncology patient and their families.

  • The navigator ensures that all patients with an oncology diagnosis receive quality care and comprehensive services within the respective disciplines.

Oversees the data collection as required by the NAPRC standards. Management of accurate and timely information for clinical decision-making.

  • Educates patients and families about the disease process, treatment options, and potential side effects.
  • He/she will coordinate patients through the continuum of care in collaboration with the multidisciplinary team.

Contact outside facility pathology departments to obtain outside slides/reports. Contact outside facility radiology departments to obtain outside imaging/reports if the images are not already in the PACs system. Capture the relevant patient details for discussion.

  • He/she will serve as the liaison between disciplines and any supportive/community services.

Monitors the patients with suspected or confirmed rectal cancer to ensure presentation at the tumor board. Navigate the patients through diagnostic and treatment pathways.

  • He/she will provide leadership within the team, and proactively develop processes that streamline and improve the clinical experience for both the patient and referring physicians.
  • He/she will work closely with marketing and outreach departments to educate referring physicians and facilities on available services, prevention, screening/detection, and treatment programs.
  • He/she will be involved in coordinating weekly conferences and database tracking analysis for treatment outcome purposes.
  • He/she will utilize protocols to refer patients to appropriate specialists and enrollment in clinical trials.
  • Educates patients and families on the transition to Survivorship and the program available at the BCC.
  • Set expectations for the post-treatment transition and remain available to patients and families for questions after completion of treatment.
  • He/she will demonstrate the knowledge, skill, and coordination to provide care and guidance to the cancer patient from screening to survivorship.
  • Assists patients with end-of-life decisions.

Active involvement in program-specific quality and selection meetings, protocol development, and educational programs for the community, patients, and staff.

Travel to multiple locations with the ULH system as needed.

Maintain compliance with all company policies, procedures, and standards of conduct.

Performs other duties as assigned.
