‍⚕️ What You’ll Do as a Pediatric RN Case Manager:

• Coordinate Pediatric Home Care – Oversee individualized care plans for children with acute and chronic conditions
• Deliver Direct Nursing Care – Administer meds, manage wounds, trachs, vents, and respond to clinical needs
• Educate & Empower Families – Provide instruction and emotional support to caregivers and families
• Collaborate with a Care Team – Work closely with physicians, therapists, and internal staff to ensure quality outcomes
• Monitor, Document & Adjust – Track progress, respond to changes in condition, and maintain clear clinical records
• Lead with Compassion – Serve as an advocate and champion for your patients and their families
  
  

What You’ll Need

• Graduate of an accredited nursing program
• At least 1 year of RN experience, with pediatric care strongly preferred
• Active RN license in the state of Connecticut
• Reliable transportation, valid driver’s license, and auto insurance
• Ability to lift up to 100 lbs. and perform physical nursing tasks
• Strong clinical judgment, communication skills, and a heart for service
