- Coordinate Pediatric Home Care – Oversee individualized care plans for children with acute and chronic conditions
- Deliver Direct Nursing Care – Administer meds, manage wounds, trachs, vents, and respond to clinical needs
- Educate & Empower Families – Provide instruction and emotional support to caregivers and families
- Collaborate with a Care Team – Work closely with physicians, therapists, and internal staff to ensure quality outcomes
- Monitor, Document & Adjust – Track progress, respond to changes in condition, and maintain clear clinical records
- Lead with Compassion – Serve as an advocate and champion for your patients and their families
- Graduate of an accredited nursing program
- At least 1 year of RN experience, with pediatric care strongly preferred
- Active RN license in the state of Connecticut
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to lift up to 100 lbs. and perform physical nursing tasks
- Strong clinical judgment, communication skills, and a heart for service
