Position Description

Job Summary

The Unit Based Educator will assess, plan/develop, implement and evaluate learning/resource needs related to orientation and continuing education needs. Coordinates, develops and presents educational programs designed to maintain, enhance and assess job performance and competencies for ICU/CCU Service employees. Support patient education initiatives as appropriate. Provides and delivers nursing care to ICU/CCU patients. Utilizing the Nursing Process, the Registered Nurse will coordinate total patient centered-care both meeting the physical, age specific, social and mental status, cultural and spiritual needs of the patients, and ensuring optimal care and service in accordance with physician orders, The Joint Commission, OSHA, nursing and department guidelines. The Registered Nurse fulfills these duties to ensure continuity and quality of care, which fosters all patients and their families' best interest and well-being.

Responsibilities

Assesses individual and collective learning needs of staff.
Revises education plan annually.
Evaluates outcomes as a result of staff education.
Coordinates preceptor program.
Actively participants in various hospital committees.
Ensures staff competencies are up to date.
Maintains resource library. Maintains documents in an organized and easy- to-retrieve manner, and ensures resource information is available to staff.
Evaluates educational resources and training.
Conducts educational needs assessments to determine educational/competency needs of the staff.
Develops and revises policies and procedures.
Coordinates remediation efforts as a result of competency or performance issues.
Acts as a department resource for clinical policies, procedures and protocols.
Maintains educational skills database and educational files for personnel.
Develops orientation curriculum.
Assigns preceptors in collaboration with Director/Assistant Manager.
Ensures preceptors are equipped with proper assessment and goal forms, and collects to review for proper documentation.
Ensures new hires attend, and successfully complete, orientation.
Informs Director of concerns regarding new orientees performance. Makes recommendations for remediation
Establishes goals for, and observes, orientees; provides orientees with feedback regularly.
Evaluates preceptors and makes recommendations for change as appropriate.
Encourages and facilitates staff members and physicians to present in- services of their choice thus making the education of the staff a team effort.
Develops own curriculum for in-services using adult learning techniques. Materials are well developed and organized.
Delivers/presents in-services and education.
Coordinates activities when utilizing external resource to conducts in- services and education programs.
Initiates and completes assessments per policy appropriately throughout each patient encounter, based on accurate and systematic data collection.
Consistently demonstrates critical thinking skills to ensure impressions and conclusions reflect status of patient at time of assessment i.e. lab results, change in patient condition, medical stability and presenting symptoms and takes appropriate action.
Obtains appropriate referrals (i.e. social service, dietary, spiritual, wound care, protective services, state and regulatory reporting requirements) based on assessment.
Functions as an integral team member, contributes to the completion of patient care activities within the team as well as completes assignments independently when appropriate.
Performs nursing procedures appropriately and according to nursing policy/procedures.
Initiates a plan of care based on multidisciplinary assessment data and always knows where to find appropriate resources. Coordinates admission, discharge and transfer activities of assigned patients and initiates referrals to meet patient needs.
Assists and/or initiates procedures and treatments in a timely manner, according to physician's orders.
Maintains open communication with healthcare team and patients and applies appropriate interventions in problem situations. Reports issues to Director/Clinical Manager.
Adapts communication style to meet age-specific needs of patient.
Is able to independently access patient information and efficiently navigate the patient clinical information systems.
Assesses level of understanding and provides individualized patient/family teaching.
Documents appropriate patient teaching, as per random chart audit by peers/Supervisor.
Documents teaching (pre-op, pre-procedure, discharge) and completes in the chart.
Initiates interdisciplinary education records for all inpatients.
Identifies the learning needs of unit personnel and students to provide for optimal clinical practice. Assists new staff members/students with procedures and/or orientation.
Makes entries into patient's record with all pertinent information in a timely manner, documentation is not left until the shift ends. Accurately documents responses to PRN treatment.
Ensures significant status changes are pertinent, appropriate and individualized.
Demonstrates competency in all medication administration, IV therapy and blood product transfusion according to hospital policy and follows the five rights of medication administration.
Recognizes unique responses to medication based upon age and condition of patient. Accurately documents all responses to medication.

Ensures patient response to medication/IV assessment site is accurately documented on 100% of the charts audited and confirms that IV solution is infusing at the correct rate.
Documents pharmacy wastage as per policy.
Proactively recognizes unclear medication orders and acts as a patient advocate.
Recognizes adverse drug reactions and reports as per established procedures. Remain current of medication issues and knowledge (i.e. side effects) and utilizes drug reference materials.
Properly disposes of sharps and pharmaceutical wastes.
Obtains, analyzes and interprets ECG Rhythm strips independently and in accordance with policies and procedures.
Recognizes abnormal rhythms, lethal rhythms and reacts immediately.
Calculates rates and measures PR, QRS and QT intervals.
Obtains blood specimens from implanted venous access devices or via venipuncture.
Uses correct specimen tubes.
Documents correct information on specimen tube label.
Delivers blood specimens to Lab in a timely manner.
Serves as a leader of the code team.
Performs all duties assigned including medication preparation, administration and defibrillation.
Performs complete and thorough documentation in the Clinical Information System.
Takes initiative to follow up and ensure that equipment is returned to the unit in proper condition.
Operates equipment accurately and safely, according to policy, procedure and or manufacturer's directions. Quickly learns to use new equipment. Uses equipment independently.
Leaves equipment prepared for next procedure i.e. cleaning and restocking as appropriate.
Obtains and interprets hemodynamic parameters independently.
Understands, and can state the rationale for, hemodynamic monitoring.
Identifies normal and abnormal waveforms without assistance.
Sets up pressure transducer system independently and according to policy and procedure.
Is able to identify ports and states function of each.
Cares for a patient with an endotracheal tube (ET) according to department policy and procedure.
Demonstrates ability to prepare patient according to protocol.
Completes documentation thoroughly, according to protocol and in a timely manner.

Works with respiratory therapy to care for patients on mechanical ventilation.
Can identify criteria for weaning.
Extubates patient with assistance from Respiratory Therapy and according to unit protocol.
Identifies possible post-extubating complications and treatments.
Performs all functions independent; seldom needs assistance.
Anticipates patient need for pain management intervention, as observed/chart review.
Collaborates with physician to develop adequate pain management strategies.
Assesses patient for pain upon admission and documents by using the 0-10 pain scale.
Reassesses patient pain per policy and documents and reassesses prior to intervention and one (1) hour after intervention and documents.
Assesses patient to provide a realistic comfort goal.
Attends mandatory in-services, staff meetings, actively participates in self-education and completes all required competency assessments on time.
Willingly performs other duties as required.

Knowledge, Skills & Abilities

Knowledge of Critical Care nursing normally acquired through specialized training or previous experience of three years. Demonstrated knowledge of staff development and program development. Excellent presentation skills, confidence, and poise in the training environment. Possesses group and team facilitation skills. Knowledge of accreditation standards, federal guidelines, healthcare organization standards and adult learning principles. Ability to communicate effectively with varied levels of staff. Effective interpersonal communication skills in order to effectively collaborate with physicians, and instruct patients and their families. Knowledge of basic computer operations and use of word- processing and presentation software.

Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs.
