Position Description

Job Summary

The LPN Graduate works under the supervision of a Registered Nurse (RN) or licensed healthcare provider to deliver patient care in accordance with established policies, procedures, and standards of care. While completing requirements for licensure as a Licensed Practical Nurse (LPN), the LPN Graduate gains hands-on experience and clinical skills in a supportive healthcare environment. This role focuses on basic nursing care, professional development, and collaboration with the healthcare team to ensure high-quality patient outcomes.

Essential Functions

  • Provides direct patient care under the supervision of an RN, including monitoring vital signs, assisting with activities of daily living, and responding to patient needs.
  • Conducts focused patient assessments, identifying and reporting changes in patient conditions to the appropriate team members.
  • Administers medications, treatments, and injections as prescribed, ensuring compliance with the Nurse Practice Act and safety protocols.
  • Assists in the development, implementation, and evaluation of patient care plans.
  • Maintains accurate and timely documentation of patient care, interventions, and observations in the medical record.
  • Ensures adherence to infection control protocols, safety standards, and healthcare regulations.
  • Assists with basic procedures such as wound care, catheterization, and specimen collection within the scope of practice.
  • Participates in the orientation and training of new staff or nursing students as assigned.
  • Collaborates with interdisciplinary healthcare teams to ensure the delivery of coordinated care.
  • Prepares and maintains equipment and supplies necessary for patient care.
  • Responds to medical emergencies and supports code team interventions within the scope of competency.

  • Emergency Services LPN Gradute:
    • Supports triage and assists with collecting patient history and vitals.
    • Prepares patients for procedures and assists RNs and providers during care.
    • Administers prescribed medications within scope.
    • Provides basic wound care and apply dressings as directed.
    • Assists in stabilizing patients under supervision.


  • OR Services LPN Graduate:
    • Prepares patients pre-operatively and assists with surgical prep.
    • Monitors patients in pre-op and post-op under RN supervision.
    • Assists with vital signs, catheter insertion, and surgical checklist procedures.
    • Provides post-operative care including dressing changes and discharge teaching under RN guidance.


  • Cardiac Surgery LPN Graduate:
    • Monitors vital signs, performs ECGs, and reports abnormal findings to RN or provider.
    • Assists with post-op care such as wound care and activity support.
    • Administers prescribed medications as allowed under LPN scope.
    • Documents patient status and support recovery milestones.


  • Endoscopy LPN Graduate:
    • Prepares patients for procedures by reviewing intake forms and obtaining vitals.
    • Assists in positioning patients and providing emotional support.
    • Monitors patients during recovery and provides post-procedure education under RN guidance.
    • Cleans and restocks procedure rooms and ensures equipment is prepped.


  • Obstetrics/Labor and Delivery/Postpartum/Nursery LPN Graduate:
    • Assists with maternal vital sign monitoring and postpartum care.
    • Provides newborn care including bathing, feeding support, and diaper changes.
    • Administers prescribed medications to mothers and newborns within scope.
    • Supports breastfeeding education and routine screenings under RN direction.
    • Assists with documentation and family education.


  • Oncology LPN Graduate:
    • Provides supportive care including medication administration, hygiene, and comfort.
    • Monitors for side effects and reports findings to RN or provider.
    • Offers emotional support and reinforces education provided by RNs.
    • Assists with central line dressing changes (if trained) and symptom tracking.


  • Orthopedics LPN Graduate:
    • Assists with mobility, transfers, and use of assistive devices.
    • Applies braces, splints, or slings as directed.
    • Provides pain medication as prescribed.
    • Monitors neurovascular status and report changes.
    • Supports patient education and discharge instructions under RN guidance.


  • Telemetry LPN Graduate:
    • Records and reports cardiac telemetry readings (but do not interpret them).
    • Monitors vital signs and provides basic care.
    • Administers medications and supports patients with cardiac diagnoses.
    • Reports any changes in rhythm or condition to the RN or provider.


  • Dialysis LPN Graduate:
    • Takes vital signs and prepares the patient for treatment.
    • Monitors patients during dialysis and report any concerns.
    • Provides comfort and reinforces post-treatment instructions.
    • Supports patient mobility and hygiene.


  • Cath Lab LPN Graduate:
    • Prepares patient for procedure (vitals, consent form verification).
    • Starts IVs (if trained and permitted), administer oral pre-meds.
    • Monitors patient post-procedure, check dressing sites, and assist with discharge.
    • Provides patient education under RN guidance.

  • Performs other duties as assigned.
  • Complies with all policies and standards.
Qualifications
  • 0-1 years of experience as a nursing assistant or other allied health role required
Knowledge, Skills and Abilities
  • Basic understanding of nursing principles and practices, including patient care techniques and safety protocols.
  • Strong communication and interpersonal skills to collaborate effectively with patients, families, and healthcare team members.
  • Attention to detail and ability to follow established procedures and clinical guidelines.
  • Proficiency in basic computer skills, including the use of electronic health records (EHR).
  • Willingness to learn and grow in a dynamic healthcare environment.
Licenses and Certifications
  • LPN - Licensed Practical Nurse - State Licensure within 90 days of employment required
  • BCLS - Basic Life Support issued by American Heart Association (AHA), American Red Cross (ARC), or American Safety and Health Institute (ASHI) required
  • NRP - Neonatal Resuscitation required
  • TNCC - Trauma Nursing Core Course required
  • NIH - National Institute of Health Stroke Scale Certification required
  • STABLE certification within 6 months of employment required
State Specific Requirements
  • Alabama: Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications required. Crisis Prevention Intervention (CPI) certification required.
  • Florida: Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Neonatal Resuscitation Program (NRP) certifications required.
  • Georgia: Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Neonatal Resuscitation Program (NRP) certifications within 90 days of employment required.
  • Indiana: CPR certification within 90 days of employment required.
  • Mississippi: Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Neonatal Resuscitation Program (NRP) certifications required.
  • New Mexico: Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Neonatal Resuscitation Program (NRP) certifications required. Trauma Nursing Core Course (TNCC) certification and National Institutes of Health (NIH) Stroke Scale certification required. STABLE certification required within 6 months of employment.
  • Oklahoma: Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Neonatal Resuscitation Program (NRP) certifications required.
  • Pennsylvania: Advanced Cardiovascular Life Support (ACLS) certification required.
