In this vital role, you will help shape the future of heart failure care through education, advocacy, clinical expertise, and program development. Nurse Navigator Heart Failure

What You'll Do

• Serve as a clinical expert, patient advocate, and educator for individuals with heart failure.
• Coordinate the care of heart failure patients from admission through discharge and support seamless transitions across the continuum of care.
• Collaborate with the Advanced Heart Failure (AHF) Medical Director, Heart Failure Program Coordinator, and nursing leadership to optimize outcomes and reduce readmissions.
• Support development and implementation of heart failure clinical guidelines and performance improvement initiatives.
• Deliver patient- and caregiver-focused education and facilitate team-based staff learning.
• Ensure compliance with DNV and other regulatory guidelines to maintain program certification.
• Serve as a liaison within the health system and in the community, representing the heart failure program at various levels.

What You Bring

Education:

• Bachelors Degree in Nursing Required. Masters preferred.

Experience:
Minimum of 3 years experience in cardiology Required

Licensure and Certifications:

• Active RN license in the State of Florida Required
• Basic Life Support (BLS) Required

Preferred Skills and Qualifications:

• Membership in professional/academic organizations
• Basic proficiency with Microsoft Office Suite (Word, PowerPoint, Excel)
