Job Title: Case Manager

Location: Morristown, NJ 07960

Schedule: 8a -4p, with weekend requirements

Duration: 13 weeks with possible extension

Pay Range: $70 to $74/Hour

Job Description:

Coordinates patients through acute care hospitalization. Manages all aspects of discharge planning/transition process. Performs utilization management and quality screenings. Inpatient Case Management is required.

Skills:

• 2 years of acute care experience strongly preferred
• Case Management, Managed Care in Patient Case Management is required.

Education:

• BSN
