A healthcare services organization in Hempstead, NY is looking to add a new Licensed Practical Nurse (LPN) to support a 1:1 assignment with a middle school student for the upcoming school year.

About the Opportunity

• Start Date: September 2, 2025
• End Date: June 2026
• Schedule: Monday to Friday
• Hours: 8:00am to 3:00pm
• Location: Hempstead, NY

Responsibilities:

• Provide 1:1 nursing support to a 6th grade student with diabetes
• Perform blood glucose fingersticks 3 times daily
• Monitor the student throughout the school day and respond appropriately to diabetic symptoms or emergencies
• Communicate with school staff, parents/guardians, and healthcare providers as needed
• Maintain accurate and timely documentation of care provided
• Ensure adherence to the student's diabetic care plan and follow-up instructions
• Support the student's safety and well-being throughout the school day

Qualifications:

• Graduate of an accredited School of Nursing
• NYS Licensed Practical Nurse (LPN)
• Comfortable and experienced working with diabetic students
• Strong assessment and clinical skills
• Patient-focused with excellent interpersonal and communication skills
• Organized and detail-oriented

Desired Skills:

• Previous experience in school nursing or 1:1 pediatric care
• Experience managing diabetes care in children
• CPR certified
