Job Overview
Performs all aspects of hospital wide performance improvement activities with special focus on coordination of stoke initiatives. The Stroke Coordinator interacts in a collaborative role with physicians, nurses and other health care professionals to advance evidence based practices related to stroke management. Plans, organizes, directs, coordinates and controls clinical and administrative functions for Stroke Management services in accordance with hospital strategic plans. Coordinator is responsible for completion of stroke measures data abstraction for both Joint Commission accreditation and State of NJ primary stroke certification. Participates in hospital wide initiatives in support of performance improvement, regulatory, and accreditation process overall. Investigates high risk occurrences, develops plan of correction accordingly.

Qualifications
Required:
- Bachelor of Science in Nursing Degree
- Must be a licensed RN in New Jersey.
- Performance improvement experience preferred
- Working Shift: Monday-Friday 8a-4p / 9a-5p
- Day Shift
- Full-Time, 37.5 Hours Per Week
- Facilitate Patient Care – Coordinate daily interactions with patients, families, physicians, nurses, pharmacists, social workers, and discharge planners to ensure seamless transplant and IEC therapy processes
- Collaborate with External Entities – Work closely with community physicians, home health agencies, insurance case managers, and laboratory personnel to maintain a successful continuum of care
- Schedule & Coordinate Treatment – Manage patient appointments, donor searches, pre-treatment testing, and eligibility evaluations to support transplant and IEC therapy planning
- Ensure Financial & Contract Compliance – Work with the Financial Coordinator to secure insurance authorizations and maintain compliance with transplant/cellular therapy contract requirements
- Educate & Advocate for Patients – Provide comprehensive education to patients, families, and donors on transplantation and IEC therapies while ensuring informed consent documentation
- Support Research & Clinical Trials – Assist with enrolling patients in clinical trials, comply with regulatory reporting requirements (FACT, CIBMTR, NMDP, IRB), and facilitate research data collection
- Maintain Communication & Documentation – Ensure accurate medical record documentation, update referring physicians and third-party payors, and log informed consents in OnCore
- Participate in Multidisciplinary Meetings – Engage in BMT conferences, financial follow-ups, and quality meetings to support ongoing program improvements
- Ensure Regulatory Compliance – Stay updated on policies and procedures from RWJUH, Rutgers Cancer Institute, and relevant agencies while maintaining training and certifications
- Promote Teamwork & Quality Improvement – Foster collaboration across departments, support new coordinator training, and contribute to process enhancements for patient satisfaction and workflow efficiency
