Position Summary

We are seeking compassionate and motivated Home Education Caregiver Nurse provide in-home education and support to family caregivers in Monmouth County, NJ. This role is vital in helping caregivers manage the physical and emotional demands of caregiving while improving the outcomes for care recipients. As a Home Education Caregiver Nurse, you’ll offer personalized guidance, connect families to resources, and ensure caregivers are equipped to provide safe, sustained care in the home setting.

Responsibilities

• Provide one-on-one caregiver education and support through home visits, focusing on health management, stress reduction, and care techniques.
• Conduct comprehensive assessments to evaluate caregiver needs and develop individualized care plans.
• Offer case management services to ensure continuity of support, track progress, and adjust plans as caregiving situations evolve.
• Establish and maintain relationships with community organizations and healthcare providers to support referrals and program growth.
• Maintain accurate and timely documentation in accordance with organizational policies and regulatory guidelines.
• Uphold professional standards of attendance, appearance, confidentiality, and communication in all interactions.
  
  

Qualifications

• Bachelor’s degree in nursing (BSN) required
• Valid New Jersey RN license
• 1–3 years of clinical or community health experience, preferably in home health, case management, or caregiver support
• Valid New Jersey driver’s license and auto insurance
• Excellent verbal and written communication skills
• Proficiency in using smartphones, Microsoft Office, and clinical documentation software
• Excellent organizational and interpersonal skills
  
  

Working Conditions/Physical Demand : Working outdoors and indoors with phone, computer and medical equipment use

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $70,200.00 to USD $159,171.19. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
