Responsibilities
As a Registered Nurse (RN) at Southpointe Care and Rehab, you will play a vital and hands-on role in providing high-quality, person-centered patient care. Your key responsibilities will include:
- Maintaining accurate and thorough daily documentation of patient care, observations, and responses to treatments, ensuring comprehensive and up-to-date patient records.
- Precisely transcribing physicians' orders onto Medication Administration Records (MARs) to ensure proper, safe, and timely medication delivery.
- Promptly notifying the pharmacy of new orders and changes to patient prescriptions, facilitating seamless and uninterrupted pharmaceutical care.
- Preparing and safely administering medications as prescribed by the physician, adhering to all established protocols, safety guidelines, and best practices.
- Collaborating effectively and proactively with the interdisciplinary team to develop, implement, and continuously evaluate comprehensive, individualized care plans that meet each patient's unique needs.
- Monitoring patient conditions closely, identifying changes or concerns, and implementing appropriate interventions in a timely manner.
- Educating patients and their families on health conditions, medications, and self-care techniques to promote understanding and compliance.
- Performing all other duties as assigned by the Director of Nursing to ensure the smooth, efficient, and compassionate operation of patient care services, contributing to the overall success of our facility.
