Title: Case Manager LPN

Type: Contract

Hours: 8:00 AM to 5:00 PM

Rate: Starting at $30.00/hour

Location: Kissimmee, FL 34759

Key Responsibilities

Ensure continuity of care using appropriate resources within contract and health plan parameters

Perform care management functions both on-site and telephonically

Serve as care team leader in collaboration with PCP and interdisciplinary team

Meet with patients, families, and caregivers to discuss care plans

Identify and follow up with high-risk patients across various care settings

Coordinate treatment plans and triage interventions with the care team

Assess, screen, triage, and perform ordered treatments

Conduct biometric screenings

Provide front desk coverage as needed, including patient check-in and phone support

Collect comprehensive clinical intake data (medication history, PMH, allergies, social history, etc.)

Review charts and records for completeness

Facilitate telehealth services, including preparing patients and equipment

Address patient needs and barriers to technology use

Process referrals and follow up with pharmacies or other entities

Prepare drug refills, lab orders, and imaging requests per protocols

Support daily patient care flow and assist in all aspects of patient care

Apply critical thinking to problem-solving and care coordination

Provide patient education during virtual visits

Track clinical orders and results

Communicate normal lab and imaging results to patients

Document assessments and procedures accurately in EMR

Use telepresence best practices to foster positive patient-clinician rapport

Provide clinical oversight and guidance to unlicensed assistive personnel

Required Qualifications

Current and valid LPN license in Florida

IV Certification

Minimum 1 year of experience in an acute clinical care setting

Experience in care management, utilization review, or discharge planning

Familiarity with HMO processes and protocols

Preferred Qualifications

Bilingual skills (Spanish/English or other languages)

Strong communication and organizational abilities

Experience with EMR systems and telehealth platforms
